IMPORTANT FORMS
Overview and Patient Eligibility
Since 1995, the Viola Startzman Free Clinic (VSFC) has provided high-quality comprehensive health and dental care, wellness education, medications, and related health services at no charge to qualifying patients. The Mission of the VSFC: “The Viola Startzman Free Clinic serves, in an atmosphere of dignity and respect, lower income uninsured residents of Wayne County.” To obtain care at the VSFC, you must:
- Be a Wayne County resident AND
- Be uninsured (including Medicaid and Medicare) AND
- Meet certain household income guidelines.
A patient care coordinator can help determine if you are eligible to be treated at the VSFC and provide you with other alternatives if you do not qualify. If you qualify for care and have never been treated at the VSFC, you may download the forms listed above, print and complete them, and bring them with you at a walk-in clinic. For a complete schedule, please refer to the information below.
Medical Care
The VSFC is open to patients five days a week and is operated by staff members and a large team of volunteers. Each week, the VSFC offers three walk-in medical clinics where patients are treated by volunteer medical professionals on a first come, first served basis. For a complete schedule, please refer to the information below. The medical staff and volunteers work together to provide patients with necessary medications and laboratory services, and hours for these services are also listed on the schedule.
If you are treated at the VSFC and a healthcare professional feels that you need additional care beyond the scope of what the VSFC can provide, you will be referred to a local specialist who can provide you with either free or reduced-cost care. We are extremely thankful to the local medical professionals who have agreed to treat our patients.
Dental Care
The VSFC dental clinic offers appointments every Thursday, 8:30 to 5:00, for routine care and two evenings a month for extraction of unrestorable teeth. Please note that you may need to wait up to three months for a dental appointment due to the high level of demand for these services. To be put on the dental waiting list, please see a patient care coordinator.
